Use the documents Carmen confirms for your planned activity. Follow the medical questionnaire’s instructions and consult a physician if directed or if you have concerns about fitness to dive.
For non-course dives, write “experience dives” in a course field only if Carmen confirms it applies and the form includes that field. Ask Carmen how to return your completed documents securely.
